What's the difference between the business, medical, and charitable rates?

The IRS sets a separate cents-per-mile rate for each purpose: business driving uses the highest rate, medical or moving miles use a lower one, and charitable driving uses a fixed statutory rate. Pick the preset that matches why you drove, since they are not interchangeable.

Can I enter my company's own mileage rate?

Yes, choose the Custom option and type any rate your employer or a non-US tax authority uses. The tool then multiplies that rate by your miles, so it works outside the standard IRS presets too.

Does it calculate in kilometers?

The presets are cents per mile, but with Custom you can enter a per-kilometer rate and treat the distance field as kilometers. Just keep your rate and distance in the same unit.
